We will cover the vacation time of the current care giver. We will follow the written daily routine to maintain and provide continuity of care.
The time period of coverage is flexible. We will write a report for the family/caregiver so they know what happened during the period he/she is away.

POST DISCHARGE CARE
TLC can help transition you from the hospital to your home. Once home we can provide the care you need to enable you to get back on your feet and into your regular daily routine: “Back to Normal”
